Hypervigilance is a state of heightened alertness in which the nervous system is continuously scanning for threat. It is characterized by increased sensitivity to environmental cues, difficulty relaxing, exaggerated startle responses, and the persistent sense that something bad could happen.

Hypervigilance develops as an adaptive response to environments where threat was unpredictable or frequent. In those environments, staying alert reduces risk. The problem is that the alertness does not turn off when the environment changes.

The cognitive cost of hypervigilance is significant. Sustained threat-scanning consumes attentional resources that are then unavailable for other cognitive tasks. Concentration is impaired. Decision-making degrades. The person is simultaneously exhausted and unable to rest.

Social hypervigilance, continuously monitoring others' reactions and emotional states for signs of threat or disapproval, is a common feature of anxiety, people pleasing, and histories of unpredictable or harsh environments.

Addressing hypervigilance requires consistent experiences of safety that gradually teach the nervous system that sustained alertness is no longer necessary. This is typically slow work that benefits from professional support.
